By Visiting the Ontario Health Center
Visiting the Ontario Health Center might sound tedious, but it’s your best shot at renewing your health card if you can’t do it online. The Ontario Government even facilitates people having any medical exemption. If you have medical exemptions and can’t visit the health center, you can fill out a Health Card Medical Exemption Request form to renew your health card online, or someone can visit the health center on your behalf.
Also, with the medical exemptions form, you need to submit all your necessary documents, such as your address proof, your identity proof, updated photograph and signature, and your health card renewal form. When you submit the form, within a few days, the Ontario Health Card Service Center sends you the current health card.

- Once you are about to turn 16 years old.
